Kashgar to Tibet & Nepal by Road

I am from Pakistan so I am planning to go Pakistan to Kashgar to Nepal via Tibet

some one guide me,
1) where to get Tibet Permit ?
2) Public Transport is available in this route?

Note that I can speak Chinese very well so language is not a problem for me.

Kashgar->using highway G-315->Yarkant->Karhilik->G219 highway starts->Rutog->Ngari->Baga->Saga Gong->Chawu-xiang->Here G-219 highway end and G-318 Highway Starts->Yalaixiang->Nylam->Zhangmu->Kodari->Nepal

China Govt make a new highway in this route. so i want to travel in this route

While using that highways/roads i’e

Kashgar / Xinjiang = G315
Rutog -> G219
Saga gong -> G219
Ngari -> G219
Chawuxiang -> G318
Rewa -> G318
Zhangmu (Nylam) -> G318

Please guide me about Public Transport / TTB Permit
